School Nurse
At Chatsworth, the health and safety of our students is a top priority. Below you’ll find important guidelines and requirements related to student wellness, medications, and state-mandated health forms. Please review the information carefully and reach out to our school nurse with any questions or concerns.
Nut-Free Policy
Do not send any food containing peanuts or tree nuts
No snacks outside of lunch periods
No food sharing between students
No food-based celebrations or parties in school
Medications at School
Medications must be in their original, unexpired pharmacy container
Proper documentation is required for any medication to be administered at school. This includes Medication Permission Forms and Emergency Action Plans for allergies, asthma, diabetes, seizures, and more.

Health Requirements
New York State requires physical exams for all new students and those entering grades Pre-K, K, 1, 3, 5, 7, 9, and 11
Please submit these required forms, prior to the start of the school year, by email to Nurse Karen Torre.
Please ensure your contact information is current in the eSchool Parent Portal.
Reminder: If your child is experiencing symptoms such as fever, congestion, or cough, please keep them home and reach out to the nurse as needed.
