School Nurse

At Chatsworth, the health and safety of our students is a top priority. Below you’ll find important guidelines and requirements related to student wellness, medications, and state-mandated health forms. Please review the information carefully and reach out to our school nurse with any questions or concerns.

Nut-Free Policy

  • Do not send any food containing peanuts or tree nuts

  • No snacks outside of lunch periods

  • No food sharing between students

  • No food-based celebrations or parties in school

Medications at School

  • Medications must be in their original, unexpired pharmacy container

  • Proper documentation is required for any medication to be administered at school. This includes Medication Permission Forms and Emergency Action Plans for allergies, asthma, diabetes, seizures, and more.

  • All required health forms can be found here.

Health Requirements

  • New York State requires physical exams for all new students and those entering grades Pre-K, K, 1, 3, 5, 7, 9, and 11

  • Please submit these required forms, prior to the start of the school year, by email to Nurse Karen Torre.

  • Please ensure your contact information is current in the eSchool Parent Portal.

Reminder: If your child is experiencing symptoms such as fever, congestion, or cough, please keep them home and reach out to the nurse as needed.
